Lab Software Installation Request Form
Please complete the form below and click Submit Form. Upon submission, the form will be reviewed and then forwarded to the appropriate dean or supervisor for confirmation.

Please note: Installation requests for Spring/May terms must be submitted by November 15th. Installation requests for Summer/Fall terms must be submitted by April 1st. One form must be completed for each unique piece of software being requested.
You do not need to submit a request for Baseline Software. Baseline software packages will be installed in every lab.

Please note: The following should be submitted to the IT Help Desk located in EJW B-32 within two working days from the time this form is submitted:
- The software itself (IT will make a copy of the CD, DVD, or other media for you to keep),
- The licensing agreement (IT will make a copy for your records), and
- Any other relevant information (purchase numbers, warranty information, manufacturer contact information, etc.) .

Images are installed in the labs twice each year; prior to the start of the Spring semester and the start of the Summer semester. Labs may not be re-imaged during a semester. For an update on the status of your request, contact the IT Help Desk at (817) 531-4428.
