Step 2. Submit your documents & transcripts
After you submit your application, we still need a few more things from you.
1. Send us your transcripts
We need to see a transcript from every college or university you've ever attended (Including the hospital where you completed nursing school if appropriate). In many cases, you can order your transcripts online. All transcripts should be submitted to:

2. Upload your resume or CV
Your resume or curriculum vita should include, at minimum, academic, work and community service experience. It can be uploaded within the application or in the self-service center.
It should also reflect evidence of ongoing education through continuing education and additional certifications.
3. Give us your references
Select three academic or professional references that provide support for your commitment to high-quality patient care, respect for human dignity, capacity for learning complex material and skills and demonstrated effectiveness as an interprofessional team member.
You'll enter their names and email addresses into the application to automate this process.
4. Prepare for your interview
A phone interview is required to demonstrate your ability to communicate a clear philosophy of primary care in a professional manner. A representative of the program will reach out to you to schedule a time.
